We hope that you will fill out our survey and help the Canadian Cancer Survivor Network by providing insights and perspectives about living with and managing advanced stages of prostate cancer. If you do not have advanced prostate cancer, please help us share the survey with others who do.
Please note that most of this survey is for patients, while there are several questions at the end for caregivers.
We will use the results of this survey to help us complete a patient evidence submission for a new medication for metastatic castration-resistant prostate cancer called Xtandi (enzalutamide). This new medication will soon be going through a national cancer drug review process at the Pan-Canadian Oncology Drug Review (pCODR) that invites patient groups like the Canadian Cancer Survivor Network prepare and send in submissions so that patients and caregivers like you can be heard. Please join us and have your voice included in this important submission process.
The survey is available online now and will remain open until March 8, 2013. Please share it with your colleagues and members of your organization – the more completed surveys we receive, the more valid our responses to the questions on our pCODR submission will be. Please note that all responses are kept private and responses on our pCODR submission will be aggregated.
You can find the survey athttps://www.surveymonkey.com/s/9HJKBQ5. If you would prefer to fill out a paper survey, email jmanthorne@survivornet.ca and we will send you the survey in a Word document, which you can complete and then email back to us.
