CCSN Questionnaires are Available for Prostate Cancer Patients

The Canadian Cancer Survivor Network currently has two feedback opportunities for prostate cancer patients and caregivers. Please read below for more information. If you qualify to take these surveys, we would appreciate you taking a few minutes to do so. If you have any questions, please contact Allison at communications@survivornet.ca.

All patients, survivors and caregivers who have faced or are facing prostate cancer are eligible to take this survey.
The purpose of this survey is to help the prostate cancer community and the Canadian Cancer Survivor Network better understand the journey that prostate cancer patients, survivors and caregivers experience during their transition from diagnosis, through to treatment, follow-up, and survivorship.

Patients with metastatic prostate cancer and their caregivers are eligible to take this survey.
The purpose of the survey is to provide the Canadian Cancer Survivor Network (CCSN) and Prostate Cancer Canada (PCC) with insights and perspectives  from patients and caregivers in order to complete a patient evidence submission for a new indication for abiraterone (Zytiga)

This survey will remain open until January 10th, 2019.
