Ces complications peuvent avoir un impact sur la mobilit\u00e9, l\u2019ind\u00e9pendance et la qualit\u00e9 de vie. <\/span><span style=\"font-weight: 400;\"><br \/>\n<\/span><span style=\"font-weight: 400;\"><br \/>\n<\/span><span style=\"font-weight: 400;\">Pour les patients atteints de cancer, il est particuli\u00e8rement important d\u2019\u00eatre attentif \u00e0 la sant\u00e9 osseuse. Parlez \u00e0 votre m\u00e9decin si vous ressentez une douleur dans les os, ou si vous pensez \u00eatre \u00e0 risque d\u2019avoir des complications li\u00e9es aux os.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\"><\/span><\/p>\n<h2><b>L\u2019origine des m\u00e9tastases<\/b><\/h2>\n<p><span style=\"font-weight: 400;\">Tout type de cancer peut se r\u00e9pandre aux os, mais certains d\u2019entre eux sont plus \u00e0 risque de se r\u00e9pandre <\/span><span style=\"font-weight: 400;\">1<\/span><span style=\"font-weight: 400;\"> :\u00a0<\/span><\/p>\n<ul class=\"content-list\">\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">cancer du sein<\/span><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">cancer de la prostate\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">cancer du poumon\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">cancer du rein\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">cancer de la thyro\u00efde<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">Environ 65 % \u00e0 75 % des patients atteint d\u2019un cancer du sein avanc\u00e9 et environ 90 % des patients atteint d\u2019un cancer de la prostate avanc\u00e9 d\u00e9velopperont des m\u00e9tastases osseuses.<\/span><span style=\"font-weight: 400;\">2<\/span><span style=\"font-weight: 400;\">\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Le my\u00e9lome multiple, un cancer h\u00e9matologique qui se d\u00e9veloppe dans les cellules de la moelle osseuse, affecte aussi les os.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Les m\u00e9tastases osseuses sont diff\u00e9rentes du cancer primitif des os et sont beaucoup plus courantes. Elles sont parfois d\u00e9couvertes avant que le cancer d\u2019origine ne soit diagnostiqu\u00e9. Cela peut mener au diagnostic du cancer primitif, mais le cancer d\u2019origine peut \u00eatre inidentifiable dans certains cas. On parle alors d\u2019un <\/span><b>cancer primitif inconnu.<\/b><span style=\"font-weight: 400;\">3<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Le cancer peut se r\u00e9pandre dans n\u2019importe quel os (dans un seul ou plusieurs), mais certains d\u2019entre eux sont plus susceptibles d\u2019\u00eatre touch\u00e9s <\/span><span style=\"font-weight: 400;\">1<\/span> <span style=\"font-weight: 400;\">4 <\/span><span style=\"font-weight: 400;\">:\u00a0<\/span><\/p>\n<ul class=\"content-list\">\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">la colonne vert\u00e9brale\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">le basin\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">le f\u00e9mur\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">l\u2019hum\u00e9rus\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">les cotes\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">le sternum\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">le cr\u00e2ne\u00a0<\/span><\/li>\n<\/ul>\n<h2><b>Signes et complications\u00a0<\/b><\/h2>\n<h3><b>La douleur osseuse\u00a0<\/b><\/h3>\n<p><span style=\"font-weight: 400;\">Un des sympt\u00f4mes les plus communs des m\u00e9tastases osseuses est une sensation de douleur. Cette douleur diff\u00e8re pour chaque personne ; pour certains, elle s\u2019intensifie la nuit alors que d\u2019autres rapportent que c\u2019est lorsqu\u2019ils bougent. Les m\u00e9tastases osseuses sont la principale source de douleurs li\u00e9es au cancer. <\/span><span style=\"font-weight: 400;\"><br \/>\n<\/span><span style=\"font-weight: 400;\"><br \/>\n<\/span><span style=\"font-weight: 400;\">Bien que la douleur soit souvent le premier sympt\u00f4me de m\u00e9tastases osseuses qui se pr\u00e9sente, certains patients ne ressentent aucun sympt\u00f4me. Environ un tiers des patients qui d\u00e9veloppent des m\u00e9tastases osseuses suite \u00e0 un cancer du sein ne ressentent aucune douleur.<\/span><span style=\"font-weight: 400;\">5<\/span><span style=\"font-weight: 400;\">\u00a0\u00a0<\/span><\/p>\n<h3><b>L\u2019hypercalc\u00e9mie<\/b><\/h3>\n<p><span style=\"font-weight: 400;\">L&#8217;hypercalc\u00e9mie est un \u00e9tat caus\u00e9 par un taux sur\u00e9lev\u00e9 de calcium dans le sang. Elle est parfois d\u00e9tect\u00e9e durant une analyse sanguine de routine, mais d\u2019autres sympt\u00f4mes peuvent aussi \u00eatre une indication, tels que <\/span><span style=\"font-weight: 400;\">4<\/span><span style=\"font-weight: 400;\">:\u00a0<\/span><\/p>\n<ul class=\"content-list\">\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">la constipation\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">se sentir l\u00e9thargique ou somnolent\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">avoir tout le temps soif\u00a0<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">D\u2019autres sympt\u00f4mes peuvent aussi appara\u00eetre avec le temps <\/span><span style=\"font-weight: 400;\">4<\/span><span style=\"font-weight: 400;\">:\u00a0<\/span><\/p>\n<ul class=\"content-list\">\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">une faiblesse musculaire\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">de la confusion<\/span><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">un coma<\/span><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">une insuffisance r\u00e9nale<\/span><\/li>\n<\/ul>\n<h3><b>Les fractures<\/b><\/h3>\n<p><span style=\"font-weight: 400;\">Les m\u00e9tastases osseuses fragilisent les os, ce qui accro\u00eet le risque de fractures. Dans certains cas, les m\u00e9tastases osseuses peuvent \u00eatre la cause de fractures spontan\u00e9es. Elles se produisent souvent dans les os des jambes, tout particuli\u00e8rement dans les os pr\u00e8s du bassin, car c\u2019est un point qui supporte la pression du poids du corps. Des chirurgies pr\u00e9ventives (telles que l\u2019insertion d\u2019une tige dans l\u2019os) sont parfois pratiqu\u00e9es pour pr\u00e9venir ces fractures.\u00a0<\/span><\/p>\n<h3><b>Compression de la moelle \u00e9pini\u00e8re<\/b><\/h3>\n<p><span style=\"font-weight: 400;\">La compression de la moelle \u00e9pini\u00e8re est une urgence m\u00e9dicale grave qui doit \u00eatre trait\u00e9e imm\u00e9diatement puisqu\u2019elle peut mener \u00e0 une paralysie permanente. <\/span><span style=\"font-weight: 400;\">5 6<\/span><\/p>\n<p><span style=\"font-weight: 400;\">La colonne vert\u00e9brale est un des endroits o\u00f9 les m\u00e9tastases osseuses peuvent se d\u00e9velopper. <\/span><span style=\"font-weight: 400;\">3<\/span><span style=\"font-weight: 400;\"> Lorsqu&#8217;une m\u00e9tastase dans la colonne vert\u00e9brale exerce une pression sur les nerfs de la moelle \u00e9pini\u00e8re, il se produit une compression de la moelle \u00e9pini\u00e8re, ce qui peut entra\u00eener de nombreux sympt\u00f4mes :\u00a0<\/span><\/p>\n<ul class=\"content-list\">\n<li style=\"font-weight: 400;\"><b>Douleur dans le dos ou le cou : <\/b><span style=\"font-weight: 400;\">Cette douleur est ressentie au lieu d\u2019origine de la tumeur et peut s\u2019intensifier avec le temps. Elle est souvent plus intense durant la nuit et lorsque la personne tousse ou \u00e9ternue.<\/span><span style=\"font-weight: 400;\">3<\/span> <span style=\"font-weight: 400;\">5<\/span><span style=\"font-weight: 400;\">\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\"><b>Faiblesse : <\/b><span style=\"font-weight: 400;\">Sensation de faiblesse ou difficult\u00e9 \u00e0 bouger les jambes ou les bras. Ce sympt\u00f4me appara\u00eet plus tard, indiquant que les nerfs commencent \u00e0 \u00eatre endommag\u00e9s.<\/span><span style=\"font-weight: 400;\">6<\/span><\/li>\n<li style=\"font-weight: 400;\"><b>Sensation d&#8217;engourdissement <\/b><span style=\"font-weight: 400;\">: Une sensation d&#8217;engourdissement dans les jambes, les mains ou le bassin. Aussi un des sympt\u00f4mes qui apparaissent plus tard.<\/span><span style=\"font-weight: 400;\">5<\/span><span style=\"font-weight: 400;\">\u00a0<\/span><\/li>\n<\/ul>\n<h2><b>L\u2019impact des m\u00e9tastases osseuses\u00a0<\/b><\/h2>\n<p><span style=\"font-weight: 400;\">Les m\u00e9tastases se d\u00e9veloppent tout particuli\u00e8rement dans les os pour plusieurs raisons : par exemple, le flot sanguin important dans la moelle osseuse, le fait que certaines mol\u00e9cules des cellules canc\u00e9reuses sont plus susceptibles de se lier aux cellules osseuses, et d\u2019autres facteurs qui rendent les os un lieu propice \u00e0 la croissance de tumeurs. <\/span><span style=\"font-weight: 400;\">2<\/span><span style=\"font-weight: 400;\">\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Un cancer qui s\u2019est m\u00e9tastas\u00e9 dans les os est toujours appel\u00e9 par le nom du cancer d&#8217;origine. Ces m\u00e9tastases sont aussi trait\u00e9es selon le lieu d\u2019origine du cancer, et non comme un cancer des os. Le cancer primitif des os, quant \u00e0 lui, est beaucoup plus rare que les m\u00e9tastases osseuses.<\/span><span style=\"font-weight: 400;\">4<\/span> <span style=\"font-weight: 400;\"><br \/>\n<\/span><span style=\"font-weight: 400;\"><br \/>\n<\/span><span style=\"font-weight: 400;\">Les os ne sont pas inertes : ils se renouvellent continuellement. Les cellules ost\u00e9oclastes d\u00e9truisent le tissu osseux et les cellules ost\u00e9oblastes le reconstruisent. Chez une personne saine, ce processus assure que les os restent r\u00e9guliers, forts, et en bonne sant\u00e9.<\/span><span style=\"font-weight: 400;\">1<\/span><span style=\"font-weight: 400;\">\u00a0\u00a0\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Les m\u00e9tastases osseuses peuvent interf\u00e9rer avec ce processus. Elles peuvent stimuler les cellules ost\u00e9oclastes ou les ost\u00e9oblastes, ce qui m\u00e8ne \u00e0 une destruction de l\u2019os ou une surcroissance du tissu osseux. Dans les deux cas, cela entra\u00eene des l\u00e9sions qui portent atteinte \u00e0 la sant\u00e9 osseuse. Les endroits pr\u00e9sentant un exc\u00e8s osseux sont tout aussi susceptibles de provoquer une fracture que les endroits o\u00f9 l&#8217;os a \u00e9t\u00e9 d\u00e9truit, car la structure globale de l&#8217;os a \u00e9t\u00e9 compromise.<\/span><span style=\"font-weight: 400;\">2 4<\/span><span style=\"font-weight: 400;\"><br \/>\n<\/span><span style=\"font-weight: 400;\"><br \/>\n<\/span><span style=\"font-weight: 400;\">Les os fragilis\u00e9s par une m\u00e9tastase sont \u00e0 risque de se fracturer spontan\u00e9ment (dans l\u2019absence d\u2019un accident comme une chute). On parle alors de <\/span><b>fracture pathologique<\/b><span style=\"font-weight: 400;\"> et ces fractures peuvent \u00eatre un signe de la pr\u00e9sence de m\u00e9tastases chez les patients qui n\u2019ont pas encore \u00e9t\u00e9 diagnostiqu\u00e9s. <\/span><span style=\"font-weight: 400;\">4<\/span><\/p>\n<h2><b>Traitements\u00a0<\/b><\/h2>\n<p><span style=\"font-weight: 400;\">\u00c0 l\u2019heure actuelle, les m\u00e9tastases osseuses sont incurables. Par contre,plusieurs traitements sont disponibles pour r\u00e9duire les sympt\u00f4mes, g\u00e9rer la douleur et am\u00e9liorer l\u2019esp\u00e9rance de vie des patients.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Deux types de m\u00e9dicament sont principalement utilis\u00e9s pour traiter les m\u00e9tastases osseuses : les bisphosphonates et le denosumab. Ils pr\u00e9viennent la perte de masse osseuse en bloquant les cellules ost\u00e9oclastes. Leur efficacit\u00e9 est maintenant bien \u00e9tablie et ils peuvent aider \u00e0 pr\u00e9venir les blessures osseuses.<\/span><span style=\"font-weight: 400;\">7<\/span><span style=\"font-weight: 400;\">\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">La radioth\u00e9rapie et certaines chirurgies peuvent aussi \u00eatre utilis\u00e9es comme traitements.
