Halton Healthcare, Patient and Family Advisory Council
Halton Healthcare is seeking patients, family members and caregivers who have received care at the hospital or one of its community service locations within the last two years, who are willing to share their experiences and perspectives to enhance patient care. The role of a volunteer advisor will influence the patient experience as well as future programs, services and policies.
The Patient and Family Advisory Council are chosen from the pool of Patient and Family Advisors. The work with this committee requires a 2-4hr commitment per month.

Halton Healthcare: Renal Patient and Family Advisory Council
The Renal Patient and Family Advisory Council (PFAC) at Halton Healthcare is comprised of patients, family members, caregivers and staff from within the Nephrology Program. The PFAC members listen to patients, family members and caregivers to improve on the patient experience. The members also collaborate and advise on services and care delivered to renal patients at the Oakville Trafalgar Memorial Hospital, the Burlington Dialysis Centre and the Independent Dialysis Program (dialyzing at home).
